Please fill out form and click on submit button at bottom of form. You will need to either email (licensing@missouri.edu) or fax (573-884-4847) the proposed design. You may also deliver the form and design directly to 319 Jesse Hall. We will respond as quickly as possible.

Once the LT Office has approved and returned this form to the contact listed above, it is the contact's responsibility to provide the manufacturer with a copy of this document for their files.

Departments and Recognized Student Organizations (RSO) must adhere to the Authorization Policy for Student Organizations and University Departments.
